Overview: The Personal Care Partnership Program is a demonstration project of the Robert Wood Johnson Foundation's Building Health
Systems for People with Chronic Illness and The Pew Charitable Trusts. In conjunction with Philadelphia Corporation for Aging,
and local personal care homes, Einstein has created a multi-disciplinary care management system for residents of personal
care homes. Care Management and primary care services are provided by a Geriatric Nurse Practitioner (GNP), in collaboration
with the Primary Care Physician. The GNP monitors the care of the resident in the personal care home working with staff and
coordinating other service providers in the network. Team-building and training of personal care staff is an integral component
of the program. A comprehensive evaluation of the clinical and financial outcomes of the program is underway, for the purpose
of providing information to key state agencies in the policy-making arena.
