Title: Access to Better Care (ABC Program)

Overview: This project focuses on the needs of the mentally-ill, developmentally delayed, and/or dually diagnosed residents in 21 personal care homes. The ABC program brings social work, chaplaincy, and congregational nursing services (through AEHN Nurse Practitioners) to these residents, adding a holistic component of care to their lives.
