
Title: Community-Based Screening of Minority Populations for Diabetes

Overview: This project seeks to demonstrate that a large urban health care provider can effectively work with community groups to design
and implement screening programs that identify minorities with undiagnosed diabetes, at a reasonable cost. The study will
accomplish this by identifying and contacting employers and community groups with minority constituencies (employees, members,
program participants, etc.) and conducting screenings and follow-ups. The effectiveness of the program will be assessed through
tracking information on the number of participants, the number of identified cases, and the number of screened positives who
access follow up health care.
