Einstein Montgomery Home Health, Hospice and Palliative Care, a service of Einstein Medical Center Montgomery, is the new name of the program that provides a full range of personalized services in the home, hospital or assisted living facility. Formerly called Montgomery Home Care, Hospice and Palliative Care, in business since 1985, the program provides a team of dedicated registered nurse practitioners, certified home health aides, medical social workers, bereavement counselors, and physical, occupational and speech therapists that provide pain and symptom management, wound treatment, personal care, emotional and spiritual support, and assistance with financial concerns.
The Home Health team provides services to help make the transition from the hospital to home as smooth as possible for patients and reduce the chances of a return to the hospital. They work with patients to achieve maximum independence and wellness in their home or assisted living community while recovering from surgery, illness or injury. Einstein Montgomery Home Health offers a Hospital-to-Home Joint Replacement Program, Congestive Heart Failure Program, Home Care to Hospice Bridge Program, and other services.
Hospice care helps patients live out their remaining time in comfort, dignity and peace. The Einstein Montgomery Hospice philosophy of care is based on respect and compassion for the entire family during a difficult time when further curative treatments are no longer recommended or desired. Einstein Montgomery Hospice provides coordinated care for people with diagnoses such as cancer; advanced dementia; failure to thrive; end-stage cardiac, pulmonary, renal or neurological illness; and other life-threatening diseases.
Palliative care services are geared for seriously ill patients or those who face chronic health problems such as symptoms associated with cancer or advanced heart, lung, liver, kidney, vascular or neurological diseases, or pain that is difficult to manage. Palliative care is provided at any point during an illness, and can be delivered at home, in the hospital, at a nursing home or in an assisted living facility.
