Richard Grant, MD, MPH has a special interest in home care. As chair of the Division of Geriatric Medicine he established a home visit program. Dr. Grant is board-certified in internal medicine with added qualifications in geriatrics. He is a Graduate of Boston University School of Medicine.

Ellen Zagrebelsky, MD, completed a residency program in primary care/internal medicine and a fellowship in geriatrics at Einstein Medical Center Philadelphia. Born and raised in Russia, Dr. Zagrebelsky is fluent in Russian and English, and can communicate in Ukrainian and Belorussian. In addition to seeing patients at the Prime Health office, she also visits homebound patients and nursing homes.

Claudine Clarke, MD, helps hundreds of older adults lead healthier lives. The connection with her patients, which often includes a hug, is part of her "whole-istic" approach to wellness. Dr. Clarke earned her medical degree at Howard University, Washington, DC. She sees patients at Einstein Medical Center Philadelphia, Klein Building, Suite 331, 5401 Old York Road, Philadelphia PA 19141.
