The medical case managers at Einstein Community Practice Center are here to work closely with you on navigating complex systems and improving your health outcomes. We can work with you on a variety of issues including:

  • Housing/utilities
  • Food insecurity
  • Transportation
  • Medication adherence
  • Keeping medical appointments
  • Immigration resources
  • Referral to additional services

If you are interested in receiving Case Management Services at Einstein, please contact the CSU at 1-800-985-2437 or 215-685-5293 to complete an intake. You can also ask for case management during your medical appointment for a more in-depth review of available services.
