Hospice & Palliative Medicine - Montgomery

Thank you for your interest in the Hospice and Palliative Medicine (HPM) Fellowship at Einstein Medical Center Montgomery. Our fellowship is an ACGME-accredited, one-year training program within the department of Family Medicine. Our goal is to train subspecialist physicians with expertise in all aspects of clinical hospice and palliative care, including complex pain and symptom management as well as advanced communication skills surrounding serious illness care. We place a strong emphasis on the importance of the interdisciplinary team in both hospice and palliative care, and trainees will be welcomed into our community of physicians, advanced practice providers, social work and chaplaincy specialists, nurses, and population health experts.

We aim to train fellows who not only provide expert whole-person and goal-concordant care to every patient, but to become leaders in the integration of palliative care into local and national health systems. To this end, our program has developed a unique focus on the intersection of palliative care and population health. As the largest acute care hospital in central Montgomery County, PA, our institution has a long history of service to the region, and our clinical service has long provided hospice care and ambulatory palliative care to our population, including a robust in-home and facility-based program. More recently, we have partnered with Einstein Care Partners, the Accountable Care Organization (ACO) that provides insurance coverage to many residents in our region. This collaboration allows us to prospectively identify patients in need of palliative care and provide this care concurrently with disease-directed treatments. As a fellow in our program, you will not only learn to provide cutting-edge palliative care to individual patients, but also to develop innovative strategies to ensure access to palliative care for all patients with serious illness in our community.
