The Philadelphia TelePrEP Program (PTP) is a new program of the Philadelphia Department of Public Health. Our goal is to make getting PrEP affirming, easy, and private by allowing you to do your visit from home and to get your labs and medicine in a way that works for you.

Step 1: Reach out to our Navigator to sign up for the program or schedule a medical appointment here.
Step 2: Our nurse will help you get labwork done at our lab in the Community Practice Center, your local LabCorp or Quest, or through a lab kit delivered to your home.
Step 3: Complete your televisit with a doctor to discuss your sexual health and all of your PrEP options.
Step 4: If you decide on a pill, then we can have it delivered to your home or you can pick it up at a pharmacy of your choice. If you decide on the injection, we can administer it.
Step 5: Once you’ve been set up with our program, then we will follow up with you every 2-3 months to make sure you get your refills and follow-up visits done on time.

The medical case managers at Einstein Community Practice Center are here to work closely with you on navigating complex systems and improving your health outcomes. We can work with you on a variety of issues including:

• Housing/utilities
• Food insecurity
• Transportation
• Medication adherence
• Keeping medical appointments
• Immigration resources
• Referral to additional services

If you are interested in receiving Case Management Services at Einstein, please contact the CSU at 1-800-985-2437 or 215-985-2437 to complete an intake. You can also ask for case management during your medical appointment for a more in-depth review of available services.

 

Your medical provider will help you decide when it is a good time to start HIV medicine. There is a pharmacist to answer your questions. Certain medications work better for certain people. Your medical provider will talk with you about which medications are right for you. Taking medications every day reduces the amount of virus in your body until it’s undetectable, which keeps you healthy and prevents transmitting the virus to someone else.

Ask about the CCN Pharmacy program that can deliver your medications to your house.

 

The IDC has a Nutrition Specialist to help patients with dietary needs, which may include:

• Nutrition counseling to help you understand the special nutritional needs related to HIV/AIDS
• Connections to food banks and meal delivery

Talk with your provider or a social worker about accessing our nutrition services.

 

The social work office is a great place to start if you need any kind of help! You can contact us by phone, during walk-in hours, or ask to speak with a social worker during your medical appointment.

The IDC has a team of social workers that assist IDC patients with a range of psycho-social needs. These services include:

• SPBP enrollment for medication and/or labs coverage
• Insurance navigation
• Supportive counseling
• Education about HIV and AIDS
• Referrals to community resources
• Liaising with medical providers and case managers
• Assistance accessing medications
• Appointment outreach
• Linkage to mental health services
• Condom distribution

Maintaining your mental health is important, and the IDC has mental health counselors who work together with an Einstein psychiatrist to provide you with the services you need to thrive. Mental health counselors can work with you on issues related to depression, anxiety, trauma, and substance use, as well as things like smoking cessation and insomnia. Leave a message at 215-456-7337 to learn more or set up an appointment with a mental health counselor.

Please note that in order to meet with an IDC mental health counselor or the IDC psychiatrist, you must be an active patient of the IDC clinic. If you are NOT a patient of the IDC and are looking for mental health services, please call the Einstein Outpatient Psychiatry Department at 215-456-9850.

 

The IDC provides comprehensive services to people living with HIV regardless of insurance status or ability to pay. The team of social workers includes an insurance specialist who is available to help patients assess eligibility and enroll in insurance programs.

Support and Informational Groups offer an opportunity to meet others, get support, learn about the clinic, and learn about HIV. The Support Group meets on the 1st Thursday of each month from 1:00-2:00pm in the 8th Floor Conference Room of the Tower Building at Einstein Medical Center Philadelphia. Lunch is provided.

Other educational groups are offered throughout the year with topics ranging from women's health to HIV & diabetes or HIV & heart disease. Contact the social work office to learn about any upcoming educational sessions.

 

The IDC Community Advisory Board (CAB) is a group that helps improve patient care. All IDC clinic patients are able to join the CAB. The CAB meets monthly to gather feedback and discuss ways to improve the clinic. Members must make at least two meetings per year to remain on the CAB.
