How to Get Started with Your First Pain Management Appointment

Getting into our system and understanding the pain management treatment process is an important step. We are here to help. Please have the following information available when making your first pain management appointment:

  • Name, address, phone numbers
  • Emergency contact information
  • Medical insurance information and member number
  • Referring physician name, phone number, address
  • Primary care physician name, phone number, address
  • Your preferred language and whether you need an interpreter during your visit
  • Other details about your condition

How to Prepare for Your First Pain Management Appointment

We look forward to discussing your pain treatment plan with you and welcome all questions. Please prepare for your first pain management appointment by bringing the following items to your first appointment:

  • Completed New Patient Intake form (PDF)*
  • Completed Medical History checklist (PDF)*

    Be sure to complete these required forms, your appointment may need to be rescheduled if either of them is missing. Keystone Mercy in particular has specific guidelines for their patients.
  • Insurance card and two forms of photo ID
  • All relevant past medical records including MRI, CT scans, bone scans and/or EMG reports (you need to bring both the films and written reports)**
  • Worker's compensation/auto insurance phone number, adjuster's name and mailing address
  • Referral or prescription slip, if required by your insurance
  • Physical/occupational/aquatic therapy discharge summaries**
  • Co-pay, if required by insurance

    Special note to Keystone Mercy patients: Your insurance requires you to have had an MRI and completed physical therapy within the last year before they will authorize fluoroscopic injection therapy. You may want to discuss these requirements with your referring physician and have them completed before your first pain management visit in order to expedite your care.

What to Expect from Your First Pain Management Appointment

Your first appointment will consist of a thorough medical history and physical examination. You will be seen by a nurse, a physician assistant and the physician. This initial evaluation may take up to an hour, because a full and complete assessment is a key component in developing your customized treatment plan.

Due to the length of this examination, and various other factors such as insurance authorizations, please do not expect medications or injections during your first visit.

Please try to arrive 15 minutes early for your first pain management appointment, and call at least 24 hours in advance if you must cancel. If you are more than 15 minutes late for your appointment it may need to be rescheduled. Pain management appointments canceled less than 24 hours in advance will be considered a no-show and after 3 no-shows you will not be rescheduled.

Is Pain Management Covered by Insurance?

It is commonly asked if pain management is covered by insurance. Einstein Pain Institute accepts most insurance plans. It is important to understand what your health insurance will pay for. Your insurer may have specific exclusions to some chronic pain therapies. Einstein Pain Institute will call your insurer to determine your coverage and any coverage limitations. We can also find out your co-pays.
